1. All patients must register at the registration counter and obtain their outpatient cards and queue numbers. They are then to proceed to their respective consultation rooms. Patients must pay for their medicine before collecting their medicine at the dispensary. They are to strictly follow the physician¡¯s instructions when consuming the medicine. Patients are not allowed to request for medicine unless they turn up at the clinic in
person.
2. Patients will be allocated in sequence to physicians depending on degree and type of illness, medical history and condition at time of registration. Patients need to pay an additional S$2.00 if they specially request to see certain physicians.
3. Patients must wait for their turn according to their queue numbers. No jumping of queue is allowed.
4. Under emergency circumstances or if the patient¡¯s condition deteriorates drastically, they will be led by the staff to the consultation rooms for immediate treatment on humanitarian grounds. This will not apply to normal patients.
5. The clinic consists of an Internal Medicine department, Acupuncture department, Gynaecology department, Paediatrics department, Dermatological department, Massotherapy department and specialist departments. The following do not fall within our treatment scope: Sexually Transmitted Diseases, Acute Immune-deficiency Syndrome (AIDS), severe burns/scalds, fractures, fighting injuries, prostration, coma and contagious diseases (e.g. smallpox, cholera, diphtheria, typhoid, leprosy, infectious hepatitis). Please take note.
6. Patients, members of the public or philanthropists who wish to contribute to the Chung Hwa Free Clinic medical fund are to obtain acknowledgement receipts from our staff. This is to stamp out embezzlement of funds. They can also deposit their cash contributions into charity boxes located within the clinic.
7. The police will be notified if a patient unfortunately passes away while at the clinic. The police or the deceased¡¯s next of kin will be in charge of the deceased¡¯s death matters, e.g. wake, funerals, etc.
8. Any imperfection in this set of rules and regulations will be looked into and corrected (if required) by the clinic¡¯s management committee. Any changes will only be effected upon the Singapore Chinese Physicians¡¯ Association executive committee¡¯s approval.
