报名费包括学术资料、茶点和午餐。与会者将同时受邀出席2012年12月16日的新加坡中医学院60周年晚宴。
报名费不包括任何医疗和旅游意外保险。

Registration fee includes academic information, tea breaks and lunches. All participants will be invited to the 60th anniversary dinner of Singapore College of Traditional Chinese Medicine on 16 DEC 2012. Registration fee does not cover any medical and travel insurance.

Please fax or email the transfer slip/proof of payment ,along with the completed registration form to Congress Secretariat. For bank draft/cheque, please indicate <ASEAN Congress> at the back of the bank draft/cheque.

  • 1. 报名确认书 Confirmation of Registration

    一旦接收付款，秘书处将于7个工作日内，以电邮或传真方式将确认书发给与会者。报名费不包括银行行政与办理费。

    This will be issued by email or fax within 7 working days upon receipt of full payment. Registration fee does not include bank administrative charge for bank draft and telegraphic transfer.


     
  • 2. 取消参会 Cancellation of Registration

    若要取消报名，请于9月30日之前以书面形式通知大会秘书处。在扣除20%行政费后，80%余款将退回给报名者；9月30日后不再办理退费手续。

    For cancellation of registration, please inform the Convention Secretariat in writing. An administration fee of 20% will be chargeable, the balance 80% will be refunded. No refund will be allowed after 30 SEP 2012.
